🧑‍⚕️ What You’ll Do as a Healthcare Assistant/Health Coach

The HCA/Health Coach is central to Doccla’s virtual ward service. You will combine remote clinical monitoring with behavioral health coaching to empower patients to manage their own health. Your goal is to build patient confidence, set personalized action plans, and ensure high-quality care, helping patients stay well in the comfort of their own homes.

Key Responsibilities

• Remote Monitoring: Utilize the Doccla platform to track vital signs and collect patient data.

• Health Coaching: Use motivational interviewing and behavior change techniques to support patients with lifestyle goals (e.g., smoking cessation, weight management, and regular exercise).

• Patient Support: Provide reassurance and comfort, ensuring patient safety and identifying potential risks.

• Clinical Collaboration: Manage care pathways, assist with audits, and escalate concerns to clinicians per established protocols.

• Administrative Excellence: Maintain accurate patient records, manage appointments, and facilitate social prescribing to connect patients with community support.

Who You Will Work With

• Patients & Families: Providing direct support and education.

• Clinical Team: Partnering with Doccla clinicians and external medical staff for joined-up care.

• Internal Teams: Collaborating with Patient Support, CQC Registered Managers, and Key Account Managers.

💡 What We’re Looking For

• NVQ Level 3 in Health and Social Care or equivalent experience (desirable)

• Current Care Certificate (desirable)

• Right to work in the UK

Essential:

• A minimum of 6 months of experience as a Health Care Assistant in a clinical setting.

• Experience in patient monitoring and basic care provision

Desirable:

• Experience in remote patient monitoring or virtual wards.

• Familiarity with digital healthcare platforms.

• Experience working in a start-up or rapidly changing environment.
